Care Plan Initiation and Caregiver Category
The RN will complete the initial assessment of the patient/client and will collaborate with the physician and family to help the client determine the assistance that will best suit his/her needs.  The RN will then generate a care plan.
Registered Nurse (RN):
The RN is responsible for nursing care and /or health care with a focus on maintenance or preventative services, in accordance with the client’s care plan.
Licensed Practical Nurse (LPN):
The LPN provides nursing care or health care with a focus on maintenance or preventative services, in accordance with the client’s care plan, under the direction of an RN in the home.
Certified Nurse’s Aide (CNA)
The CNA is responsible for administering services that are necessary for the maintenance of the patient in the home.  The Nurse’s Aide provides personal care assistance with activities of daily living - such as bathing, grooming, toileting and meal preparation – assistance with ambulation, assistance with medical equipment and routine health monitoring.
Caregiver in all three categories may include:
- light house-keeping, laundry, meal preparation
- grocery shopping taking the client to appointments
- services consistent with the maintenance of a safe and supportive home
